Eve sinned first. It was the sin of pride that led her to eat the apple. Adam is the father of sin. 

(2) Was death and sin present in the world and universe before the fall of Adam & Eve?

No

Satan

(4) Was Adam made and formed 'mortal'? 

Adam was mortal before he sinned and ate the apple. He was immortal after he sinned and ate the apple.

1.) It sounds to me like Eve was the first to sin (Genesis 3:6). I've never heard the expression "father of sin" before.

2. First, let us define "death." It would appear to me to have two meanings. It can mean "the end of existence." It can mean "to suffer." But biblically speaking, I would say that death and sin did not exist before the fall of Adam and Eve. However, the account could be symbolic.

3. Is this a trick question? I'm guessing you are referring to Satan.

4. If "mortal" means "destined to die," I would have to say yes. I don't see where this is heading.

Satan sinned first, but Adam brought sin into this world

Death and sin in the world before the fall of Adam:    yes and no....    no if the tree of life had been kept available to Adam   BTW Eve was deceived so sin was attached to Adam.

Satan

Adam was formed as a perfect human who would have been given access to the tree of life had he not sinned....   without the tree of life he lived 700 to 900 years and died.

Adam was made in the image of God, since God is immortal so was Adam an immortal soul-spirit.

What died in Adam the day he rebelled was his communication with God, before his rebellion God talked with him and He with God, ...they met together "in the cool of the evening" to converse together.

-Death in Adam, Life in Christ-

 ..."Therefore, just as sin came into the world through one man, and death through sin, and so death spread to all men because all sinned—  for sin indeed was in the world before the law was given, but sin is not counted where there is no law.  Yet death reigned from Adam to Moses, even over those whose sinning was not like the transgression of Adam, who was a type of the one who was to come.
