The short answer is that they married their sisters or cousins/close relatives. The gene pool was not corrupted like it is today and so marrying one's sister didn't have the problems associated with such an act in our day. Well, it is important to remember that conditions were different and people were living to be 800, 900 years old and so the ability to have lots of children was pretty great. Humanity was reproducing in greater numbers than we realize. 80 years old at that time, was not like 80 years old today. A woman's children bearing years would have lasted for centuries in her life given how long they were living. So by the time people Cain took his wife, there were many, many people on the planet. There were no doctors, science researchers, if somebody got sick or injured or ate something poisonous or whatever... As one said, there was no Law... Adam and Eve might have had children with their children as well as bothers and sisters and cousins and nieces and nephews having children. What is so serious about incest is that we all have errors in our DNA, by mating with distance relatives (which we all actually are since Noah and Mrs. Noah and further back to Adam and Eve are the progenitors of us all) it reduces the likelihood of similar / precise defects in the DNA producing the mental and physical defects in children of incest. This is why we really screwed up with test tube babies, sperm banks, surrogacy, etc. and we give each privacy rights that can actually cause incest unawares. Not to mention with the aid of Eharmony and sites like it exacerbating the problem by matching candidates, with the common likes and traits in potential mates being more heredity than personal preference... what a mess.... mankind always tries to tamper with what God creates or prescribes...
