Actually, that has nothing to do with olam.   and the context clearly indicates that "forever and ever" in that verse means exactly what it says.  Those in the lake of fire will be tormented forever and ever, eternity after eternity after eternity without end.

It was most likely written in Greek but with a Hebrew mindset.   There is no reason it would have been originally written in Hebrew.  It was sent to only seven churches as far as we know and Greek was what was spoken in Asia Minor at that time.    I have no use for Catholic websites.

Idioms are figures of speech and in this case it is drawn from scriptures.  He is using the language of Isaiah to make a point about hell.  He is not literally referring to what is being described in Isa. 66.  He is drawing from it.  It was probably a cultural idiom that the people would have recognized.
