Let's put it this way - if John has a kidney transplant and he gets his new kidney from David who died in a car crash, has John really had a kidney transplant or has David had an entire body and head transplant? Well, it's kind of obvious that it is John who has had the kidney transplant. David is dead, only his kidney lives. If I was beheaded and you kept me alive by giving me a new head that was donated by Sarah, it would be Sarah's head that has now been attached to my body. Because Sarah's head, Sarah's face, Sarah's brain, Sarah's personality and Sarah's lifetime memories would live on - it would make sense to say that it was Sarah who had been kept alive and not myself.
